Individual CEU Application Form

If you have attended a LIVE lecture, that has not been approved for respiratory care ceu’s you can request CEU approval.

Print the application and mail it, along with the supporting documentation, to the CEU Evaluator.

If you are wanting to offer a group CEU and do not have a CEU Providership you can request approval. The fee is $50 per CEU.
